• All patient and family member balances are due before office visit.
• Same day scheduling is based upon availability.
• Medication refills require an office visit.
• Walk-in visits are for urgent care only.
• Some insurance companies require a physician to be selected as the patient’s primary care provider. Those patients must select Dr. Robert Gilbert as their
  primary care physician for Premier Urgent and Family Care to provide medical services.
• Form fee of $25.00 for up to two pages and $50.00 for three or more pages is due prior to completion.
• Children under the age of 16 must be accompanied by an adult at all times.
• Children under the age of 16 cannot be left unattended in the waiting room.
• Premier Urgent and Family Care does not tolerate dishonesty, profanity or threatening behavior.
• Premier Urgent and Family Care reserves the right to limit the number of patient medical issues addressed in a visit.

No Show Policy

• All missed appointments are considered No Show and will be charged a $35.00 No Show fee. Appointment cancellations must give 24 hour notice or are No Show.
• Patients arriving 20 minutes late will be considered No Show.

Referrals

• All specialist referrals are provided after an office visit.
• Referrals will not be given once you are at the specialist office.
• Approval number from your insurance must be obtained prior to making any special testing appointments
